Title: Liability Waiver Agreement Type: Required
I certify that I am the parent or legal guardian for my child(ren). I hereby give my permission for any supervisor, coach or other team administrator associated with the Emmaus Aquatic Club/Steel Fitness Premier to seek and give appropriate medical attention for our child(ren) in the event of accident, injury, illness. I am aware of the risks associated with any of the Emmaus Aquatic Club programs and the facilities, including but not limited to: the pools, pool deck, tennis courts, locker rooms, common areas,doors, diving board, equipment, diving blocks, steps, foliage, insects, gas grills, sidewalks, parking lot and surrounding areas. These risks include but are not limited to: ear and eye infections, colds, abrasions, cuts, nosebleeds, burns, stings, rashes, head injuries, injured or broken limbs, paralysis, and even death. I will be responsible for any and all costs associated with any necessary medical attention and/or treatment.
I hereby waive, release and forever discharge the Emmaus Aquatic Club/Steel Fitness Premier from all rights and claims for damages, injury, loss to person or property which may be sustained or occur during participation in the Emmaus Aquatic Club activities, whether or not damages or loss is due to negligence. I hereby acknowledge that my child(ren) is (are) physically fit and capable of participation in all Swim School activities.

SwimAmerica lesson participants enter through Steel Fitness Premier. Please check in at the front desk upon arrival.

Lesson participants should never contact Steel Fitness Premier for information regarding lessons. The contact information for the EMAC SwimAmerica program is emacswimschool@rcn.com or 610-965-5800.

No swimmer may enter the water or pool deck until called by their lesson coach or supervisor

Swimmers must be able to listen and wait their turn for the safety of themselves and others

Placement and advancement in a group will be at the discretion of the SwimAmerica staff

SwimAmerica reserves the right to cancel any group classes that are not filled and to reassign swimmers to another group class

SwimAmerica may combine levels to form a class
-
Classes must have the minimum enrollment requirement in order to be held

Enrollment of classes is on a space available basis

Due to pool availability and the complexity of scheduling, SwimAmerica does not offer make-ups/refunds for missed classes

All classes cancelled by EMAC will be rescheduled

Goggles are recommended for Levels 2 & up (none that enclose nose)

Long hair should be tied back or maintained in a swim cap
-
One piece swim suits are recommended for girls

Double swim diapers are required if not toilet trained

Remove band-aids, gum and jewelry

No one should swim with open sores, contagious skin conditions or illnesses

Parents/guardians are not permitted on the pool deck and should remain in the seating area

EMAC SwimAmerica lesson participants may not play/practice in the pools before or after their scheduled lesson time

Although we strive for consistency, requests for a specific lesson coach are not guaranteed

Circumstances may require the need of a substitute lesson coach

SwimAmerica lesson participants are NOT permitted in the Steel Fitness Premier adult locker room facilities.
Title: Payments/Refunds Agreement Type: Required
Payments:

Lessons will not be scheduled without payment of the $20 non-refundable ANNUAL registration fee per student

Lesson payment must be made in full prior to the class start date, whether by credit card or check
Cancellation Policy/Changes:

After you have registered, lessons may be rescheduled to a different day/time as long as there are openings. If you withdraw from class prior to the start of all the classes for that session, you will receive a full credit on your account (less the non-refundable $20 annual registration fee).

If you withdraw from classes after the session start date, you will be charged for all the scheduled classes up to that date whether attended or not, plus a $40.00 cancellation fee per student. Any remaining credit will be placed on your account. Credit placed on your account may be used for future lessons.

Private and semi-private lessons require a 12-hour cancellation notice by sending an email to emacswimschool@rcn.com and by leaving a phone message with EMAC at (610) 965-5800. It is also suggested that you obtain your instructor's contact information. If EMAC is not notified prior to the lesson, it will be forfeited and no make-up/credit will be given.

· Title: Photos and Videos Agreement Type: Required
· I understand that photos and videos are occasionally taken at SwimAmerica and that any photo or video taken of my child(ren) may be used for SwimAmerica purposes, including, but not limited to, our website, Facebook, print brochures or newspaper advertising for SwimAmerica. Please contact the SwimAmerica supervisor if you do not want your child to be included

Title: Inclement Weather Policy Agreement Type: Required
The Emmaus Aquatic Club's SwimAmerica program may cancel lessons for weather-related reasons. In the event classes must be canceled, an email will be sent to all class participants to your account email address. EMAC will do everything it can to reschedule any canceled classes. Please do not call Steel Fitness Premier. As we are an outside vendor at that facility, they will not know whether lessons are being held or not.
